

Do you often feel you can't stop eating once you start, or eat far more than you meant to? This 8-question screen is the eating module of the Patient Health Questionnaire (PHQ), developed by Robert L. Spitzer, Janet B.W. Williams and Kurt Kroenke. It asks about loss-of-control eating and about ways of trying to avoid weight gain over the past three months, then tells you whether your answers meet the PHQ's screening criteria for binge eating disorder or bulimia nervosa. It takes about two minutes.

Has this been as often, on average, as twice a week for the last 3 months?
In the last 3 months have you often done any of the following in order to avoid gaining weight?
Made yourself vomit?
Took more than twice the recommended dose of laxatives?
Fasted — not eaten anything at all for at least 24 hours?
Exercised for more than an hour specifically to avoid gaining weight after binge eating?
If you answered “No” to all of questions 4–7, answer “No” to this question as well.
If you checked “YES” to any of these ways of avoiding gaining weight, were any as often, on average, as twice a week?
